Founda offers a secure and fast Patient Identifier Cross-Reference Manager to help healthcare providers with their cross-referencing challenges.
When to use: An example scenario in which the cross-reference manager can be useful is when there are different patient identifiers across different systems. Let's say a patient is registered in the hospital's system with identifier 123, but in a questionnaire application with identifier abc. Based on a set of similar patient demographics, the Founda Health Platform will store a mapping from identifier 123 to identifier abc, this way both systems can use their own identifiers for identifying the same patient.
The interaction consists of two parts:
- First a Patient Identity Feed is created, consisting of patient identifiers that are cross-referenced across multiple systems. It's also possible to revise, remove or resolve duplicate identifiers.
- Once there is a mapping of patient identifiers, it can be used to retrieve the identifier needed by querying the patient identifiers from Founda's Patient Identifier Cross-Reference Manager.
